Proxy Voting Form
NOTE: The name, address, cell phone and email address for ALL new Executive Representatives, Tournament Representatives and Team Captains must be sent to the 1st VP by October 1 of any given year. The 1st VP will update the Board Roster immediately.
If a Board member is unable to attend a MWDGA Board Meeting, that member may appoint another Board member as their voting proxy. There are two methods that can be used which are outlined below.
1. Print and complete the form below and send it with the Board member who has been designated the absent member's proxy. Present this form to the secretary at the meeting.
2. Any Board member using email as a proxy must email it to the secretary at least 24 hours prior to the Board meeting from the email registered to that member. The designee must also be copied.

The Secretary will verify the sender is a Board member and is sending the proxy from the registered email address.
